Job Description

Position Summary:

  • Under the general direction of the rehabilitation director, provides physical therapy services to patients following the plan of care established by a physical therapist.

Essential Functions:

  • Essential Functions:
    • Under the supervision of a physical therapist, provides physical therapy services, reassessing the patient’s status on each visit and ensuring quality of care as evidenced in the clinical record.
      • Implements the plan of care (POC) as established by the physical therapist.
      • Instructs patient and caregiver in treatment programs, precautions and equipment use as necessary and documents patient/caregiver demonstration of instructions provided.
      • Maintains frequency of visits as established on physician’s order or POC.
      • Documents patient status and progression towards achievement of outcomes on each visit in measurable parameters.
      • Able to ask appropriate questions of patient/caregiver and collect data necessary to monitor patient’s status.
      • Modifies interventions in accordance with patient changes, within the scope of the POC.
      • Reports changes in patient’s status and updates on progression of POC to the supervising physical therapist or rehabilitation director.
      • Collaborates with physical therapist and patient in ordering equipment necessary for patient safety and optimum function.
      • Demonstrates safe techniques in providing patient care, as observed by the director and other care providers.
    • Coordinates patient care with other clinical team members ensuring optimal patient care and communication as evidenced in the clinical record.
      • Makes recommendations to and participates in the revisions to the plan of care.
      • Utilizes appropriate community resources.
      • Recommends and participates in case conferences.
      • Updates other team members of changes in patient status that will impact on the plan of care.
      • Identifies the need for and makes recommendations for referrals to other disciplines.
    • Maintains and enhances personal knowledge as well as promoting professional standards in physical therapy.
      • Maintains current license and submits timely evidence of renewed license.
    • Completes all required documentation following treatment session to ensure that documentation reflects care provided and the outcomes achieved, as evidenced in the clinical record.
  • Demonstrate Standards of Behavior and adhere to the Code of Conduct in all aspects of job performance at all times.
Qualifications:
  • Required:
    • Graduate of an approved associate degree program as a COTA
    • Currently licensed in Washington State
    • One year experience in a hospital or rehabilitation facility preferred
    • NBCOT certified OTA
    • Must be currently certified in BLS and recertified every 2 years.
    • Must possess basic computer skills related to Windows navigation, mouse usage, accurate keyboarding, email communication and password management
