Job Description

Basic Purpose of The Job

Under the direction of the Home Care Services Director/Clinical Manager/Clinical Supervisor performs the specific clinical duties of the Homecare/Hospice Nurse, including case management of comprehensive services. Assesses individual and family health needs, develops and supervises a plan of care, performs skilled nursing duties and tasks, teaches health management concepts, and consults appropriate resources within the organization and community related to Hospice and Palliative Care.

Job Requirements

Minimum Education

Degree/Diploma from accredited school of nursing, BSN preferred.

Minimum Work Experience

At least one year of recent successful acute care experience.

Homecare and Hospice experience preferred.

Required Licenses/ Certifications

Current state registration as a professional nurse-RN.

CPR Certification.

Current valid driver’s license; availability of auto in good working condition with current insurance and willingness to drive in all types of weather.

Required Skills, Knowledge, and Abilities

Proficient competent clinical skills and the ability to exercise independent judgment as well as seek guidance when indicated.

Effective teaching and communication skills.

The ability to function autonomously in a non-structured environment.

Critical thinking and multi-tasking skills.

Emotional stability to function effectively in unpredictable situations.

Must possess basic computer skills related to Windows navigation, mouse usage, keyboarding, email communication and password management.
