Job Description
Position Summary:

Under Direction of the Home Care Services Clinical Supervisor and Clinical Manager, performs the specific clinical duties of the Home Care Nurse. The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include assessing patient and family health needs, following the plan of care, providing treatments within the LPN scope of practice, and working collaboratively with all members of the Interdisciplinary Team to meet positive patient outcomes.

Qualifications:
Required:
Graduate of an accredited practical nurse or vocational nursing program
One year of recent, successful acute care experience. Home Health and/or Hospice experience preferred.
Current valid driver’s license; availability of auto in good working condition with current insurance and willingness to drive in all types of weather.
CPR Certified
Desired:
Proficient competent clinical skills and the ability to exercise independent judgment as well as seek guidance when indicated.
Must possess basic computer skills related to Windows navigation, mouse usage, keyboarding, e-mail communication and password mgmt.
Effective teaching and communication skills
Possesses the ability to function autonomously in a non-structured environment
Emotional stability to function effectively in unpredictable situations.
