Working collaboratively with our multidisciplinary care partners, we provide case management and discharge planning for a busy 200 bed regional hospital. Our rural setting, serving an area of 12,000 square miles, provides challenging and rewarding opportunities to significantly impact the lives of residents living in North Central Washington.

Seeking a Manager for a care management department of 30 employees. Our robust Care Management Team is made up of Nurse Case Managers, Medical Social Workers, Discharge Coordinators, and Care Management Assistants. Our team assignments align with our geographically assigned Hospitalist colleagues to promote continuity and effective communication. Must maintain primary residence in Confluence Health service area.

Basic Purpose Of The Job

The Performance Improvement Manager is responsible for day to day oversight of Quality programs, staff, and initiatives as assigned by the Quality Director.

Job Requirements

Minimum Education

Graduation from program of nursing; AA required, BSN preferred.

OR Bachelor of Science in health care field, or Masters in a related or complementary field (not applicable for supervision of CDI program).

OR Masters of Social Work with LICSW credentials. Consideration will also be given to a MSW with an LSWAIC credential with commitment to qualify for and pass the LICSW exam within an agreed-upon time (not applicable for supervision of CDI program).

Minimum Work Experience – General

5 years acute care experience required.

Demonstrated expertise in case management, critical care and/or emergency department highly desirable

Previous supervisory experience preferred

Utilization Management and hospital case management experience preferred

Minimum Work Experience CDI Specific

Clinical Documentation Improvement (CDI) experience required (for oversight of CDI program only)

Required Licenses/ Certifications

Current WA state RN license required

CDS certification preferred (Clinical documentation improvement only)

Accreditation in nationally recognized professional case management organization desired

Demonstrated skills in the areas of negotiation, communication (verbal and written), conflict, interdisciplinary collaboration, management, creative problem solving, and critical thinking. Knowledge of healthcare financing, community and organizational resources, patient care processes, and data analysis.

Organizational and analytical writing and interpersonal skills

Knowledge of pathophysiology and disease processes

Familiarity with Windows based software programs (including Microsoft Office programs)

Knowledge of Medicare Part A and B

Knowledge of Medicare Inpatient and Outpatient criteria

Knowledge of regulatory environment

Familiarity with Up-to-Date and other widely accepted clinical resources

Understanding of hospital Value Based Purchasing program and impact on hospital operations

Familiarity with Clinical Core Measures preferred

Achievement of 70% or greater score on the Clinical Assessment test administered through JA Thomas/Nuance

Knowledge of ICD-9CM and ICD-10 coding rules and guidelines

Demonstrated ability to construct non-leading and effective documentation clarification in accordance with guidelines established by the AHIMA Practice Brief and JA Thomas’ Best Practice Framework

Familiar with E/M documentation and coding requirements
