For example, you may receive updates related to your visits, MyChart account, one-time passcode, billing notifications, prescription reminders, and care management. Our use of text messaging to communicate with you is subject to this Policy and the Terms and Conditions for the Confluence Health Text Messaging Program.

Surveys

From time-to-time, we may request information from you via surveys. Participation in these surveys is completely voluntary and you may choose whether or not to participate and therefore disclose the requested information. Survey information will be used for purposes of improving our health care services that we provide to patients.

Opt-Out Procedures

Users may opt-out of receiving future e-mails or mailings from Company by contacting the Company at the address or telephone number provided below.

Children and Privacy

The Company’s Website does not target and is not intended to attract children under the age of 13. The Company does not knowingly solicit personally identifiable information from minors through our Website.
