With MyChart you can use any computer or mobile device to renew your prescriptions, schedule or request appointments*, check lab results, view health information, send messages to your care team, view/pay your bill, and more.


Virtual Visits for Patients Via MyChart

Confluence Health is now offering virtual visits via MyChart. This option is available to for patients who have a mobile device with a camera and microphone, or have a camera and microphone installed on their desktop or laptop computers.

MyChart Proxy Access

MyChart proxy access enables a parent, legal guardian or other caregiver to access a patient’s health record. Adults may also grant access to another adult. Different types of proxy access are available depending on the patient’s age and circumstances.

Children aged 0-12
Parents and legal guardians with proxy access to children ages 0 through 12 will have full access to their child’s record.
This will automatically transition to limited proxy access when the child turns 13 years old.

Teens aged 13-17
If your child is 13 or older, your teen can grant either limited proxy access or full access through their own MyChart account.
Proxy access will be automatically terminated when the teen turns 18 years old.
For teen patients with severe cognitive impairment, please discuss the possibility of full proxy access with your primary care provider at a clinic visit.

Adult patients may invite through MyChart another adult, such as a spouse, to have full proxy access. For adults with limited capacity to manage their own healthcare, proxy access may be granted to another adult that has legal documentation that stipulates legal guardianship of the patient.

You may have been given a 10-digit activation code at your last doctor’s appointment. If so, click below to register online. No need to worry if you don’t have an activation code. Get the care you need even faster with Fast Pass
MyChart Fast Pass is a waitlist function that notifies you if an earlier appointment becomes available to help you get the care you need faster. If there has been a cancellation and you have requested an earlier appointment, MyChart will send you an email. Patients who respond first will receive the earlier appointment.

Schedule an appointment online through MyChart
We offer online scheduling through MyChart with the following departments. Dermatology, Family Practice, Internal Medicine, Orthopedics, Pediatrics, and Physical Therapy. Simply log in to MyChart, then select “Visits” and “Schedule an appointment.” Choose a type of visit, then select a provider and practice location to see current available appointments.

Message your doctor
The Message My Doctor feature enables you to send a secure message regarding a non-urgent medical concern to the providers that you are established with and receive a response from a member of the healthcare team within two business days.

Share Your Medical Record with Any Provider, Anywhere

Share Everywhere is a new way for you to share your medical information with the people who are taking care of you. Using your MyChart or MyChart mobile account, you can generate a share code and provide it to the person you want to share your health data with. This might be a doctor, chiropractor, physical therapist, dentist, or school nurse, for example. The share code recipient enters that code and your date of birth on the Share Everywhere website to receive one-time, temporary access to your health information. The person who views your information can also write a note back to your health system to help keep your care team informed of the care they provided.

Accessing your health information on MyChart is even more convenient with the MyChart app. Whether you’re at work, running errands or heading out to a doctor’s appointment, MyChart is available for Apple® or Android™. Once you’ve downloaded the MyChart app to your phone or tablet, simply select your health care provider and enter your MyChart username and password.
