MyChart is your secure online connection to your Confluence Health care team — available anytime, from your phone, tablet, or computer. It’s the easiest way to stay involved in your care between visits.

MyChart Fast Pass is a waitlist function that notifies you if an earlier appointment becomes available to help you get the care you need faster. If there has been a cancellation and you have requested an earlier appointment, MyChart will send you an email. Patients who respond first will receive the earlier appointment.

New MyChart Users

There are three easy ways to sign up for MyChart:

Register Online

You may have been given a 10-digit activation code at your last doctor’s appointment. If so, click below to register online. No need to worry if you don’t have an activation code. You can still register for MyChart by providing a little bit of information about yourself. Click the links to register with or without an activation code.

Download the MyChart App

Accessing your health information on MyChart is even more convenient with the MyChart app. Whether you’re at work, running errands or heading to a doctor’s appointment, MyChart is available for Apple®opens in a new tab or Android™opens in a new tab. Once you have downloaded the MyChart app on your phone or tablet, simply select your healthcare provider and enter your MyChart username and password.

Routine Care

Under the guidance of your primary care provider, our new inhouse Routine Care team guides you through necessary preventative care, enabling testing and results without waiting for a doctor’s appointment.

On-Demand Virtual Care

Confluence Health is pleased to announce that we have partnered with KeyCare to provide telehealth services (virtual visits) to our patients for minor or routine medical issues. When you need care quickly but don’t need an emergency room, virtual visits are a great option. These services are available 24 hours a day, 7 days a week, 365 days a year.

Care Companion

Care Companion is an interactive, digital assistant that helps you manage your own health, wherever you are. Care Companion is available for MyChart users on smartphones, desktop computers or tablets and through the MyChart page on our website.
