Whether care is provided at home or in a facility, Confluence Health is committed to helping patients live their final days with peace and respect.
A Team Approach
Hospice care brings together a dedicated team of professionals and supporters to care for both patients and their families—physically, emotionally, and spiritually.
Medical Care
Focused on comfort, dignity, and quality of life, hospice medical care prioritizes pain and symptom management rather than curative treatment. Patients receive coordinated care from hospice physicians along with regular visits from skilled hospice nurses.
General Inpatient Care
When symptoms become difficult to manage at home, short-term inpatient care is available. This level of care allows patients to receive intensive support in a hospital setting until their symptoms are stabilized and they are able to return home.
Therapy Services
Physical and occupational therapy services help patients maintain independence, mobility, and comfort for as long as possible, supporting overall quality of life.
Medical Equipment and Supplies
We provide essential medical equipment and supplies related to a patient’s condition, including items such as hospital beds, wheelchairs, oxygen, and medications—delivered directly to where the patient resides.
Personal Care
Hospice aides offer compassionate, hands-on assistance with daily activities such as bathing, dressing, grooming, and personal hygiene—helping patients feel comfortable and cared for.
Respite Care
Caring for a loved one can be physically and emotionally demanding. Respite care provides temporary relief for primary caregivers, offering up to five days of care in a hospice facility so caregivers can rest and recharge.
Volunteer Support
Our trained volunteers offer meaningful support through companionship, friendly visits, help with errands, and providing caregivers with a much-needed break—all contributing to a supportive, connected experience.
After-Hours Support
Support is available whenever you need it. Our triage nurses are on call 24 hours a day, seven days a week, providing guidance, answering questions, and helping manage concerns outside of scheduled visits.
Bereavement Support
Care doesn’t end with a patient’s passing. We offer grief support and counseling services to family members and loved ones, helping them navigate loss with compassion and understanding.
