Charity Care

Confluence Health offers a Charity Care Program to patients based on family size and federal poverty guidelines. Please mail or fax your completed application to Confluence Health (including all documentation). Be sure to keep a copy for yourself.

It is the policy of Confluence Health to provide equal access to its facilities and services without unlawful discrimination on the basis of race, color, national origin, disability, age, sex, sexual orientation, gender identity or expression, creed, religion, marital status, veteran or military status, inability to pay for medically necessary evidence-based care or for urgent or emergent services, insurance coverage under Medicare/Medicaid/CHIP, or any other status protected by law.   • Households that are underinsured and uninsured and whose household income* is less than 300% of the U.S. Department of Health and Human Services (HHS) Poverty Level will be considered.
  • Only “medically appropriate services” are eligible.
  • Charity care will only be considered after all other financial resources available to the patient have been explored and exhausted.
  • Nominal fees are not charged to patients at or below 200% of the federal poverty guidelines on qualifying charity care services.
  • Patients must reapply every six months for charity care reevaluation.
  • Our charity care schedule is based on the federal poverty guidelines and is updated annually.

*Household income means a group of two or more persons related by birth, marriage or adoption who live together. All such related persons are considered members of one family.

  • Patients who live within the United States who feel they may be eligible may apply for the Charity Care Program.
  • The application must be filled out in its entirety and returned within 30 days.
  • Each patient will have an initial determination done prior to sending the application and will be reviewed for other programs prior to the sending of the Charity Care application.
Definitions

Income: Total cash receipts before taxes derived from wages, salaries, welfare/social security payments, strike payments, unemployment, disability benefits, child support/alimony and net earnings from business and investment activities paid to the individual.

Household: A group of two or more persons related by birth, marriage or adoption who live together. All such related persons are considered members of one family.

Final Determination

Confluence Health will notify the applicant of its final determination within fourteen (14) days of receipt of the application and all documentation material. A patient may appeal a denial of eligibility for Charity Care by providing additional verification of income or family size to the patient services manager within thirty (30) days of receipt of notification.

Charity care and/or financial assistance means medically necessary hospital healthcare rendered to indigent persons when third-party coverage, if any, has been exhausted, to the extent that the persons are unable to pay for the care or to pay deductible or coinsurance amounts required by a third-party payer based on the criteria in Confluence Health’s Financial Assistance Policy/Charity Care Policy (which is available in four languages at the bottom of our Charity Care page).

Financial assistance and charity care shall not be restricted by service area but is available to all patients seeking medical services at Confluence Health.

Confluence Health provides an application process. Each patient will have an initial determination and will be reviewed for other programs prior to receiving an application.
