Confluence Health wants to hear from you!

All of our patients are invited to take a short survey to help us better understand how you are using MyChart, apps, text messages and other online tools to manage your health needs. In particular, the survey will give us insights into what is working well, where you are getting stuck or frustrated, and what you most want us to improve next. Your responses are completely anonymous and will only be shared in aggregate to guide improvements to our digital platforms.
