Confluence Health’s, home care & hospice services provide you with individualized medical care in the comfort and security of your own home.
We work with physicians, patients and their families to provide care that promotes healing and supports independence. Care is coordinated to meet the unique needs of patients and their families.
Home Health provides quality clinical services to homebound patients in their homes, assisted living facilities or adult family homes. Home Health team members care for patients recovering from illnesses, accidents or surgery and provide assistance to those with a disability or chronic illness. Our staff provides a full spectrum of services, ordered by a physician, including:
- Pre-Surgical Care and Evaluations
- Rehabilitation Therapy
- Cardiac Care
- Maternal Child Health
- Infusion Therapy
- Ostomy/Wound Care
- Pain and Symptom Management
The Home Health Care Team includes:
- Registered nurses who provide skilled nursing care developed for each patient
- Certified Nursing assistants who help with personal care and other activities
- Physical, Occupational and Speech Therapists who work to increase patients’ mobility, safety, independence and communication skills
- Medical Social Workers who identify resources for patients, assist with problem solving and provide short term counseling
Hospice is a concept of caring that focuses on comfort, rather than curative treatment. We offer a comprehensive and coordinated program of care honoring the unique physical, emotional and spiritual needs of each child, adult and family member based on the patient’s wishes and family’s needs. Pain and symptom management is paramount in our care.

Hospice affirms life and regards dying as a normal process. The goal is to assist patients and families to attain the necessary preparation for a death that is compassionate, peaceful and dignified.
